Wellness Nurse

The Wellness Nurse primary focus is on promoting the health and well being of the residents at New Dawn Memory Care. The Wellness Nurse is responsible for accommodating residents' preferences and for improving the health of residents as much as possible through diet, exercise, medication, PT and activities. The Wellness Nurse monitors the health of the residents, keeps records of changes in the residents health and provides medical care to residents who need it. The Wellness Nurse is responsible for educating the staff and family members. The Wellness Nurse also communicates with friends, family and all health care professionals in order to coordinate all care related to esridents.

 

Employee will:

• Be required to stand 6-8 hours at one time for

 as much as 10 hours in one day.

• Sit 2-4 hours in a day

• May be required to drive 30 minutes to two

 hours in one day

• Be occassionally required to drive up to six

 hours in one day for meetings but may take

 other form of transportation if necessary

• Be frequently required to lift and carry up to 50

 pounds for up to 30 yards

• Be occasionally required to bend, squat,

 kneel, climb and reach up to 3 feet overhead

 

Qualifications:

• A current Arizona nursing license

• CPR/First Aid Certified

• Demonstrate knowledge of nursing practices,

 techniques and methods applied to health and  wellness in compliance with federal, state and

 local regulations

• Experience in home health, assisted living,

hospital and long term care setting

• Caring and compassionate

• Minimum 1 year experience with older adults

• Supervisory experience preferred

• Competent at passing, monitoring and

 documenting medication administration

• Positive and outgoing personality

• Written verbal and communication skills are

 needed in order to effectively interact with

• Administer medication using the Medication

Administration Record.

• Ensure confidentiality or resident medications

 and personal information among all residents,

 and visitors. Following HIPPA regulations.

• Complete resident treatments as outlined in

 the Treatment Administration Record.

• Maintain order and cleanliness of medication

 cart and refrigerator.

• Restock medication/treatment cart with cups

 and spoons after all medication passes

 are completed

• Supervise Resident Assistants on shift

• Assist in formulating New Dawn health and

 wellness policies

• Monitor all refrigerated medication every

 24 hours.

• Take an active role and have a voice in family

 care plan meetings.

• Notify Director of Health and Wellness of any

emergency situations—as stated in New Dawn

 Memory Care policies

• Complete incident reports and maintain

 Incident Report Log.

Director of Dining Services

 

Reports To:  Executive Director and Regional Director of Dining

Supervises:  Assistant Cooks and coaches Resident Assistants

The Residents and Their Families

• Ensure that the New Dawn Philosophy

 including Alfa’s Guidelines are demonstrated

 by all who enter the Home.

• Know each resident and their needs through

 the Individualized Service Plan, regular visits,

 at least a monthly review of the resident

 record and review of the Daily Log.

• Work with the team to implement the New

• Dawn Risk Management and Quality

• Assurance programs.

• Key participant in the resident assessment

 process and in ongoing assessments an

 changes in the individual service plan.

• Adapt the menu other key parts of the dining

 services program in order to delight

 our customers.

• Cater to guest meals, special events, birthday

dinners, etc.

 

The Dining Program

• Plan and create the monthly menu and

distribute to the Home mailing list, residents

 and staff.

• Assist in meeting any and all regulatory

 requirements and developing any plans of

 correction as needed.

• Create a par level supply list and order and

 maintain all food and supplies for the dining

services program staying within budget.

• Establish a strong, ongoing program including

 recruitment, administrative processing, training

 and recognition.

• Work with the Program Coordinator on food

 related items for special events such as

 birthdays, holidays, parties and other events.

• Look for new techniques, methods and

 materials to enhance the dining

 services program.

• Promote the hydration program for residents

• Promote interaction of the residents in the

 dining services program such as allowing them

 to help set the table, prepare part of the meal

 as appropriate, etc.

• Work with the Director of Health and Wellness

 to develop a plan for any resident with weight

 loss or excessive weight gain, eating or

 chewing problems or nutritional or

 hydration issues.

• Attend all required care conferences.

• Work with nursing to complete and maintain.

 diet cards and food preference sheets.

• Follow up with the family or legally responsible

 party within 72 hours of the move-in, and then

 30 days after move-in to provide and update

 on the resident and ensure customer

 satisfaction. Ensure that the DCR has followed

 up with any and all appropriate staff if

 improvements are required.

 

Human Resources

• Creates a positive, caring, helpful, flexible

 and fun work environment.

• Teach and demonstrate the New Dawn

philosophies of caring, promoting the best

 day possible for the resident and

 customer service.

• Recruit new staff members to maintain fully

 staffed department.

• Responsible for training and coaching

• Routinely observes the Resident Assistants in

Director of Health and Wellness

Reports to:  Executive Director

Supervises:  All Nursing staff including LPNs, RNs and Medication Technicians

 

The Director of Health and Wellness works closely with the Executive Director, nurses, QMAPS, doctors and all health care providers to ensure that all of the Residents needs are met. The DHW establishes positive relationships and communication with families and outside medical personnel for the resident’s wellbeing. The DHW works to maintain the resident in their New Dawn Memory Care home and to continually improve the quality of care and the reputation of New Dawn Memory Care.

Responsibilities

• Conduct pre-admission assessments in a

 timely manner with the goal of reaching and

 maintaining all occupancy goals.

• Manage the medication program including

 working with the pharmacy, pharmacy

 consultant, QMAPS and physicians, training

 and supervising the nurses and QMAPS,

 monitoring the medication documentation and

 all other tasks related to proper medication

 administration.

• Assist in the recruitment, training, scheduling,

supervising and coaching of the nursing and

 QMAP team including performance appraisals.

• Communicate with families and responsible

parties any health and wellness related issues

 as per the New Dawn Memory Care policies,

 while maintaining positive relationships.

 Coordinate and communicate with physicians,

 rehab groups, home health, hospice and other

 health related services on resident's behalf.

• Serve on-call 24/7 for health emergencies

 unless a designee has been assigned to cover

 with permission of the Executive Director.

• Visit residents in the hospital or other settings

 as needed.

• Participate in the professional networking,

 particularly to other health care providers to

 educate them about New Dawn Memory Care

 and to develop referral relationships.

• Provide the staff with ongoing training and

 coaching particularly as it relates to health and

 wellness and managing difficult behaviors.

 Assist in the start-up opening of New Dawn

Memory Care Sun City West including the

 selection of start-up supplies within budget,

 organize workspace, establish policies and

 procedures and assist in the initial licensing of

 the community.

• Prepare the home for regular license

 inspections with other members of the

 management team.

• Promote the New Dawn Memory Care

philosophy, Residents Rights, ALFA

 Philosophy of Care and the Alzheimer’s

 Association core principles.

• Participate in the Community’s operating

 budget process as requested by the Executive

Director as related to the Health and Wellness.
